Job Summary

Essential member of the Nurse Family Partnership (NFP), an evidence-based, nurse home visiting program that improves the health, well-being and self-sufficiency of low-income, first-time parents and their children. Provides comprehensive community health nursing services to women and their families eligible for the NFP Program, maintains the highest standards in clinical nursing practice, adheres to the NFP model, and to policies, procedures, guidelines and standards of NFP and of the Prisma Health System. Self-directed and accountable for the coordination of care of assigned cases. Adheres to all compliance, policy and procedure requirements of the organization including, but not limited to licensure renewal, assigned training, employee health screenings, time and attendance policy, dress code policy, patient confidentiality, infection control, medication administration. Demonstrates knowledge of regulatory agency requirements (JCAHO, DHEC). Provides care within the scope of the SC Nursing Practice Act.

Essential Functions

  • Assess the need for and availability of equipment and supplies for the provision of patient care.
  • Identifies and communicates to the NFP Nursing Supervisor those areas where improvements and change may be necessary.
  • Demonstrates the ability to organize and prioritize assignments.
  • Completes assigned tasks in a timely manner as delegated by the NFP Nursing Supervisor.
  • Maintains confidentiality of patient information and adheres to HIPPA regulations.
  • Develops therapeutic relationships with women and their families in a home visiting environment.
  • Performs home visiting in accordance with the NFP model and guidelines.
  • Assesses physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains.
  • Assists women and their families in establishing goals and outcomes.
  • Provides education, support and referral resources in assisting women and their families in attaining their targeted goals.
  • Consults and collaborates with other professionals involved in providing services to women and families.
  • Engages in skill building to meet all NFP competency requirements.
  • Meets with NFP Nursing Supervisor weekly for clinical supervision.
  • Utilizes reflective practice in supervisory sessions.
  • Schedules joint home visits with NFP Nursing Supervisor every four months.
  • Attends and participated in bi-weekly case conferences.
  • Attends and participates in bi-weekly staff meetings.
  • Provides information to support staff for timely and accurate data input to the NFP CIS.
  • Cooperates in review and analysis of the CIS reports for achievements and areas for improvement.
  • Participates in quality improvement efforts.
  • Documents patient information appropriately and accurately.
  • Understands, supports and coaches others in the NFP vision, mission and model.
  • Represents the NFP vision, mission and model in actions and verbally to both internal and external customers and colleagues.
  • Assists in creating a positive work environment that promotes productivity, mentoring, teamwork and cooperation.
  • Elicits and considers differing viewpoints when analyzing issues.
  • Recognizes accomplishments of team members.
  • Participates in the planning of education programs for staff.
  • Assists with providing NFP educational programs to other health care professionals, community agency staff, parents and teachers.
  • Completes all NFP required education.
  • Successfully achieves reliability with NCAST Feeding and Teaching Scales.
  • Utilizes computerized data entry equipment and various word processing, spreadsheet and file maintenance programs to enter, store and/or retrieve information as requested or otherwise necessary and summarizes data in preparation of standardized reports.
  • Inputs NFP data in a timely and accurate manner into the web-based information system; complies with or assists in the compilation of statistical information for special reports.
  • Demonstrates behavior that facilitates the daily operation of the program.
  • Stays informed of current health care developments to provide safe, quality nurse home visiting services.
  • Maintains clear lines of authority, responsibility and communication by following appropriate chain of command to assist in resolving issues, discussing concerns and addressing problems.
  • Demonstrates flexibility to needs of the program accepting changes in assignment and schedule based on needs.
  • Identifies and progresses toward meeting personal goals.
  • Interacts positively with children, family members and other professionals in a respectful, courteous and cooperative manner.
  • Maintains satisfactory attendance and follows established policies and procedures.
  • Professional appearance is expected and adherence to the dress code is followed.
  • May participate in community outreach activities such as distributing program brochures, retrieving client referrals as delegated by the supervisor.
  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Associate degree in Nursing (AD N)
  • Experience - One (1) year in Nursing

Required Certifications, Registrations, Licenses

  • Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
  • A valid driver’s license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and proof of auto insurance.
